Patient referrals

Referrals can be sent to our practice by mail, fax or email.

If you are registering and arranging an appointment on behalf of the patient (for example, a person in residential care), please download and and fill out the forms below. Consent must be signed by the patient’s medical and financial decision makers before an appointment can be made.

We will contact the patient and send them the registration form and medical questionnaire. If the referral documents are incomplete, we may need to contact you for further information.
