We see adult patients and young people (age 16 and older) transitioning to adult services. Our patients benefit from specialist management due to their level of frailty, disability, or the degree of complexity of their management. They include people living with:

- Cerebral palsy, stroke, and acquired brain and spinal injuries
- Older people with frailty
